MT. WEDA PIONEER MENGALAMI TROUBLE ENGINE AKIBAT KEBOCORAN OLI KE SILINDER // MT. WEDA PIONEER EXPERIENCES TROUBLE ENGINE DUE TO LUBRICATING OIL LEAK INTO CYLINDERS

Senin, 9 Desember 2024 – Kapal MT. Weda Pioneer mengalami insiden Trouble Engine (main engine) selama pelayaran dari Tanjung Priok menuju Weda. Berikut kronologi kejadian berdasarkan laporan dari Departemen Mesin:

Kronologi Kejadian:
Pada pukul 17:54, saat kondisi pelayaran normal, terdengar suara hentakan mendadak seperti seseorang menahan napas. Beberapa detik kemudian, mesin utama mati secara otomatis dalam waktu 3–4 detik.

Setelah mesin utama mati, pihak kapal segera menginformasikan kondisi ini kepada anjungan dan Kepala Kamar Mesin (KKM) agar Master kapal dapat memantau situasi pergerakan.

Proses Inspeksi Awal:
Tim teknis melakukan pemeriksaan visual pada mesin utama dan mempersiapkan turning gear untuk memeriksa apakah ada sumbatan pada piston atau crankshaft. Selama proses turning, ditemukan indikasi adanya kebocoran minyak pelumas (lubricating oil) ke dalam ruang bakar silinder No. 1, 6, dan 7, dengan oli menyembur keluar dari katup indikator (indicator cock). Sementara itu, dari silinder No. 4, hanya air tawar pendingin yang keluar.

Tindakan Penanganan:
Setelah proses turning selesai, diadakan rapat internal untuk menganalisis penyebab kebocoran oli dan menentukan langkah-langkah penanganan. Dalam koordinasi dengan Technical Department Office, langkah awal yang diambil meliputi:

  1. Penggantian silinder No. 4.
  2. Overhaul injector pada silinder No. 1, 6, dan 7, termasuk mengganti O-ring holder injector menggunakan spare part yang tersedia di kapal.

Proses pergantian komponen selesai dilakukan dengan lancar.

Penyelidikan Lanjutan:
Hingga saat ini, penyelidikan mengenai penyebab pasti masuknya oli pelumas ke ruang bakar silinder masih berlangsung. Laporan ini disusun untuk menjadi dasar evaluasi dan tindak lanjut lebih lanjut dari pihak terkait.

Kesimpulan:
Insiden ini menyoroti pentingnya pemeliharaan rutin dan deteksi dini pada Main Engine kapal untuk mencegah gangguan serupa di masa yang akan datang.

Monday, December 9, 2024 – The MT. Weda Pioneer experienced a main engine breakdown during its voyage from Tanjung Priok to Weda. Below is the chronology of events as reported by the Engine Department:

Chronology of Events:
At 17:54, during normal sailing conditions, a sudden thumping noise was heard, resembling a sharp gasp. Within 3–4 seconds, the main engine shut down automatically.

After the shutdown, the bridge and the Chief Engineer (KKM) were immediately informed to allow the Master to monitor the vessel’s movement and situation.

Initial Inspection Process:
A visual inspection of the main engine was conducted, followed by preparations to use the turning gear to check for blockages in the piston or crankshaft. During the turning process, lubricating oil was observed spraying forcefully from the indicator cocks of cylinders No. 1, 6, and 7, indicating oil leakage into the combustion chambers. Meanwhile, only cooling freshwater was observed leaking from the indicator cock of cylinder No. 4.

Action Taken:
Following the turning process, an internal meeting was held to analyze the cause of the oil leakage and determine the necessary actions. In coordination with the Technical Department Office, the following initial steps were taken:

  1. Replacement of cylinder No. 4.
  2. Overhaul of injectors for cylinders No. 1, 6, and 7, including replacing O-ring injector holders with spare parts available onboard.

These replacement and repair actions were successfully completed.

Further Investigation:
As of now, investigations are ongoing to determine the root cause of the lubricating oil entering the cylinder combustion chambers. This report has been prepared for evaluation and further follow-up by relevant parties.

Conclusion:
This incident highlights the importance of routine maintenance and early detection to prevent similar issues in the future.
